Creatinine, urea, eGFR, haemoglobin, electrolytes — ideally the last three sets so trends are visible.

Protein, albumin-creatinine ratio and microscopy where available.

Kidney ultrasound or CT reports, with images where they can be shared.

Biopsy and specialist letters
Histopathology and nephrology clinic letters give the clearest picture of cause.

Every current medicine with dose and how long you have taken it.

A short written history
When symptoms started, hospital admissions, and what you have been told so far.
